  • Minor surgical procedures
  • PAP test
  • IUD/Implant insertion or removal
  • Wellness baby/child check-up
  • Complete Annual physical examination
  • Driver's Medical exam
  • Completion of forms(Please note that a fee will be charged for completion of the Drivers Medical Form as well as for any other Forms.)
